Documents Required :
  1. Original Welfare board I.D. card
  2. Copy of Ration card
  3. Copy of police FIR
  4. Letter from Police Station concerned that the missing fishermen is not yet traced
  5. Family Member certificate
  6. Letter consent from other legal heirs for disbursing the relief amount to the applicant in Rs.20/- stamp paper (only from legal heirs aged 18 years and above)
  7. Indemnity bond (Letter consent for refunding the relief assistance sanctioned to the Missing fishermen when he will be traced out)
  8. Certificate from concerned ADF &FW / Revenue Department regarding no relief assistance was sanctioned from CM relief fund and from any other Government schemes
  9. Copy of Nominee Bank pass book
  10. Detailed incident report from concerned ADF &FW
